Early registration is advised. Registration
must be accompanied by correct registration payment. Confirmation will be mailed
after completed registration form and payment are received two weeks before conference
date.

All parking is free.
There
will not be any partial credit of contact hours. Registrants must attend the entire
day(s) to obtain contact hour certificate.

- Fax the completed 2008 conference registration form, with completed credit card information, to 904.713.4859. Fax registrations are accepted 24 hours a day. Please be sure that the registration form is filled out completely before submitting via fax.
Purchase orders
must be received by the conference date or payment will be required by the individual
on site.
All cancellations must be sent to Deborah Brabham in writing at the address above. Cancellations must be postmarked by May 16.
Cancellations are subject to a $25.00 processing fee.
There will be no refunds for cancellations
received after the conference begins or for “no shows.”
